4、如果申请CQC认证，同时也申请CB测试证书，请填写完“申请人承诺”后，继续填

写CB测试申请书。

5、CQC认证与CB认证同时申请时，制造商必须相同。 6、CB申请书以英文填写为准，相同的内容可以复制。
